Our Support Model
ElLemuel Charitable Foundation is a newly established organization. At this time, only our Intake & Assessment service is fully operational. All other services are planned and will be implemented gradually as funding and operational capacity allow.
Intake Assessment
We begin with a standardized intake and consent process, including income and residency verification, as well as assessments to determine each participant’s needs and readiness.
Eligibility Screening
We use fair and transparent screening protocols based on verified low income (using the Low-Income Measure), residency, and demonstrated need.
Service Navigation
Participants receive guided access to resources such as housing support, legal aid, and mental health services through caseworkers and a community resource directory, with follow-up included.
Outcomes Tracking
We monitor participant progress in areas like housing, employment, and financial literacy through surveys, anonymous reporting, and impact storytelling for continuous improvement and accountability.
Case Management
Personalized action plans are developed to support each participant’s goals, including help with forms, applications, and ongoing connection to mentorship and relevant services.
Supportive Follow-Up
Participants benefit from 30-, 60, and 90-day check-ins, access to a support hotline, and, where funding allows, childminding, transportation assistance, and trauma-informed mental health workshops.
